SLIDING FEE
For those individuals without insurance, we offer a sliding fee scale based upon income and family size. In order to enroll you must provide proof of income. If you are eligible for Medicaid, our social workers can help you apply.
You may also qualify as a self payer if you have only catastrophic coverage, or the service you have is not a covered service.

It is important to note that if you are part of a managed care plan and your primary care provider is someone other than a provider here at the Center, your services may not be covered. The name of your primary care provider will be on your insurance card if you have managed care.
APPOINTMENTS
Appointments may be made for all programs by calling 603-447-8900 and pressing "1". General appointment hours are from 9:00 am to 4:00pm Monday through Friday. Please bring your insurance card with you.
As there is a demand for appointments, it is important that you keep yours. If you cannot, as a courtesy please give 24 hours notice.
