BECOME A PATIENT

We are so glad you’re considering becoming a patient of White Mountain Community Health Center!  Call us to register as a patient, or just stop by when we’re open: 8 am – 4 pm, Monday – Friday. If you call after hours, leave a message with a number we can reach you at during business hours.  You can also email us at info@whitemountainhealth.org to start the process, just be aware that email isn't HIPAA secure.  You can request a secure email, however.

Here are some things to know about your first visit:

  • Before you can schedule a visit, we'll need you to fill out the registration forms below and return them to us.  Stop in if you want a printed version.
  • We'll want your medical records before your appointment, if possible.  Fill out the medical records request form on the last page of the new patient packet, and give it to your most recent medical provider.  Let us know if you need help with this process.
  • A few days before your first visit, you'll get a text or email prompt for self-check-in.  This will allow you to enter some additional information ahead of your appointment.
  • Come 15 minutes early to your first appointment to complete your patient registration paperwork.  Please bring your health insurance card, a list of your medications and supplements or the bottles, and notes about any health issues you want to discuss at your appointment.
  • Get in touch if you have any questions or need help with the registration process.  We're happy to answer questions or have someone sit down with you to complete the paperwork.
  • To register in a language other than English, email Care Coordinator Cheryl Frankowski at cfrankowski@whitemountainhealth.org in your preferred language.  She can help you register and schedule an appointment with interpretation assistance.
