Feedback
We welcome feedback on our services! We rely on input from patients to make improvements and ensure that we provide high-quality, compassionate care to every person who walks through our doors.
Did you have a good experience?
We would love to hear about it! Writing a Google Review helps others find out about our services and quality of care and is a great way to thank a staff member who you particularly appreciate. You can also share your appreciation privately through our feedback form.
Do you have a question or a concern?
We always want to know if your experience with us didn’t match your expectations, so we can figure out what went wrong and make it right. Our Quality Improvement and Assurance Committee reviews every patient grievance and uses them to improve our services. Contact us directly so we can be in touch and resolve the issue or answer your questions.
Do you have a healthcare story you want to share with more people?
We’re always looking for patient stories that show the need for the individualized community health services we offer, the effect of policy changes on the people we serve, and the ways we make a difference in people’s lives. If you have a story you’d like to share, please use our feedback form and share your contact information so we can reach out if we have any questions.
